Case Manager/Social Worker (Hospital/Long Term Care)
SUMMARY

The Case Manager provides oversight and intervention through assessing, planning, coordinating, monitoring and evaluation of services required to respond to individual resident/patient needs. The Case Manager is a hybrid position coordinating plans of care at both Webster County Community Hospital and Heritage of Webster County.

This position complies with department and organization-wide policies, critical access hospital (CAH) and long-term care (LTC) facility requirements, safety, and infection control, and all Federal and State of Nebraska regulations related to the performance of Webster County Community Hospital and Heritage of Webster County operations and requirements of the compliance program.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Responsible for utilization review, discharge planning and care coordination with the resident/patient, resident/patient’s family, provider and health care team.

Coordinates activities that promote quality outcomes and resident/patient throughput while supporting a balance of optimal care and appropriate resource utilization.

Performs a comprehensive assessment of psychosocial, medical and discharge needs of residents/patients and their families along with an assessment of resources appropriate and available to the resident/patient and family.

Documents professional recommendations, discharge plan, care coordination interventions, and case management activities to effectively communicate to all members of the health care team.

Utilizes proper body mechanics, transfer/lifting techniques and appropriate equipment to minimize fall risk to resident and injury to self.

EDUCATION AND/OR EXPERIENCE

Case management experience required.

Social work degree preferred.

Strong knowledge of state and federal regulations governing critical access hospitals/nursing homes.

Commitment to providing compassionate and resident-centered care.

CERTIFICATES, LICENSES, REGISTRATION

Certification in Case Management required.

Current Basic Life Support (BLS) certification required.
